Innovaccer Unveils AI Solution to Cut Hospital Readmissions and Improve Care Efficiency

The solution integrates Innovaccer’s Data Activation Platform with predictive analytics, automated care management workflows, and agentic AI tools to deliver smarter, more scalable care transitions. Hospitals using the platform can proactively identify high-risk patients, streamline care coordination, and reduce exposure to costly penalties under CMS’s Hospital Readmissions Reduction Program (HRRP).
